His mother had him evaluated when he was about 9yo by an audiologist who specializes in auditory processing disorders. Turned out he has "auditory integration deficit". One of the results of this is that auditory information never really makes it to the part of the brain it needs to get to for processing. This boy also had the excessive movement issue you are describing and had difficulty learning to read.

This boy is now 15yo and doing great in high school. His mother pulled him out for a year to do remedial work with him, and therapies. (They did PACE and Phono-Graphix.)

Interestingly enough, when he was 9yo this mother also took her son to be evaluated by a very highly respected neurologist. This doctor basically told her he was ADHD and needed to be on medication. When she decided not to take his advice about medication, he told he she'd be back. In this boy's case, though, the other therapies were enough to reduce many of his symptoms -- enough, anyway, that he can function very well in the real world.

If you are looking for recommendations, I would suggest starting with an occupational therapy evaluation and a developmental vision evaluation. If you can rule out problems in these areas, you will have narrowed the range of possibilities a *lot*.
