What is a sweet, sentimental type gift for a first time mother?

My brother and his wife are expecting their 1st baby in December. They just found out they are having a boy! I would like to do something sweet for them and I just can't think.

I'm sure there are all sorts of new little thingy's out there that would hold sentimental value.

Any ideas? Not sure if I'm thinking of something for the baby or for the mommy.
