I only occasionally watch this show, but I've seen her do it this way--
She establishes a bedtime routine that's not long and drawn out. Then, she has the parents say goodnight, give kisses and leave the room. First time the child gets up, they gently lead the child back to his/her room with a very gentle, "It's time to go to bed, Sweetheart. You need to stay there and sleep. Goodnight, Darling." One kiss, and then leave. Second time and every time thereafter, parent just silently and gently leads the child back, puts in bed without saying a thing, and closes door (door was closed before, too).
From what I've heard, it's the consistent, silent, and gentle manner that tells the child you mean business.
